The Headache Question

Chronic symptoms of the head and neck can often be attributed to:

Headache — the temporalis muscle (it closes and clenches the jaw)

Sinus pressure and pain — the lateral pterygoid muscles (it moves the jaw side to side and/or forward)

Neck stiffness and pain — trapezius muscle (it stabilizes the skull during jaw clenching and grinding)

Dental offices have treated and helped more and more people with their headache problems. For years, we assigned all of these names to headaches, like muscle tension headaches, neuralgia, migraine and so on, and it seems that many headache patients share one very common trait- They clench or grind their teeth at night!

Most medical research has shown that headaches, even people with classical migraine headaches, have no physical reason, no vascular problems and no neurological problems; in fact their physician’s exam will give no physical reason for the pain. Many patients have had CAT scans and MRI’s that were negative, and find that drugs really don’t help their problem; instead the medication makes them groggy and “drugged out.”

What we have discovered is that people who can control their nighttime clenching and grinding will get tremendous relief for their headaches and neck aches. Many people do so much unconscious clenching of their jaw muscles that when they wake up, their teeth are sore, their muscles are already tired, and they are set up for the beginning of a headache from the start of the day, if they don’t wake up with one.

One effective treatment utilizes an NTI appliance (short for nocioceptive trigeminal inhibition), a dental device that fits between the upper and lower front teeth. A traditional dental mouthpiece, or splint, reduces the resistance to side-to-side movement, thereby, reducing the effort and resultant strain to the jaw joint and sinuses (so long as clenching intensity isn’t too intense). However, the same splint also provides an ideal clenching surface, where maximum clenching intensity may increase and/or allow jaw joint problems to perpetuate.

Many patients run the gamut of the medical world’s attempts to control their headaches- with multiple drugs, injections and so on, without ever thinking that the pain might be muscular in origin. But just like back pain is often muscle spasm, the pain we call TMJ, as well as headaches of many sorts are very much caused by overuse of the muscles of closing the jaws.

A Toast To Good Health With Green Tea

Civilizations around the world have used natural herbs and plants to treat sickness and pain. All tea comes from a plant, Camellia sinensis. Unlike black tea, green tea is not fermented, so its active ingredients remain unaltered. Green tea’s protection comes from a powerful antioxidant, a polyphenol called EGCG. [Graham HN. There is evidence to show that green tea can be effective in the prevention and treatment of certain types of cancer, high cholesterol, cardiovascular disease, rheumatoid arthritis and impaired immune function.

Because our mouths are an oxygen-rich environment closely connected to our blood vessels, they provide an ideal habitat for the growth and rapid proliferation of cancer cells. Scientists have confirmed that green tea not only halts the growth of new oral cancer cells but it actually breaks down and kills existing oral cancer cells. A double-blind study of people with leukoplakia (a precancerous oral condition), showed that those in the green tea group compared to those in the placebo group had significant decreases in the pre-cancerous condition. This is why we examine your mouth closely on each visit to determine any changes in texture or color that might indicate the presence of oral cancers. This early screening is just one more reason to make sure you don’t miss your regular checkup.

Ingredients in green tea may reduce the risk of getting dental cavities. One study compared two groups. The one that rinsed each night with an alcohol extract of oolong tea leaves had significantly less plaque formation than the group who did not. [Otake S, Makimura M, Kuroki t, et al. Anticaries effects of polyphenolic compounds from Japanese green tea. Caries Res 1991;25:438-43.]

Another benefit of green tea is that it stunts the growth of odor causing bacteria, thus helping you maintain a fresh breath.

And just how much green tea should we be drinking? To fully obtain the benefits, we should have at least four to six cups a day. And if you don’t want to drink it down, simply use it as a mouthwash. Decaffeinated tea is recommended to reduce the side effects associated with caffeine, including anxiety and insomnia. Supplements, in the form of tablets are also available.

What Are Dental Implants?

Dental implants offer people an alternative to the traditional ways of replacing missing teeth. The actual implant is an artificial root [anchor] made from synthetic material, usually titanium metal. There are three phases to the implant process.

First, the dental implant is surgically placed into the jawbone. It takes 3-6 months to fuse with the bone [called osseointegration]. An abutment [post] is attached to the implant and protrudes above the gum tissue. A replacement restoration is cemented or screwed to the implant abutment. Depending on the situation, dental implants can support a fixed crown or bridge or act as a stabilizing base for a full denture. The procedure can take up to 9-12 months for completion and has a high degree of success.

Some individuals have had so much bone resorption [loss] that the remaining bony ridge is too thin to hold an implant. In many cases, synthetic or natural bone can be grafted [added] or grown to allow for dental implants as an alternative treatment.

Implants have a great advantage for people already wearing full dentures since they can support and stabilize the denture while minimizing further bone loss of the denture ridge.

Not everyone is a good candidate for dental implants. There are certain risk factors that may limit success including smoking, excessive alcohol consumption, chronic bruxism [grinding teeth], systemic problems such as diabetes and individuals with poor oral hygiene.

Dental implants offer a “second chance” to those who have lost all of their teeth. For people missing only one or several teeth, dental implants provide benefits as an alternative way to restore your mouth. To determine if implants are for you, a clinical examination, x-rays, study casts and other appropriate records and measurements will be necessary. Does My Dental Insurance Pay?

If you have dental insurance coverage, like most people, you want to know how much of the treatment will be covered and how much out-of-pocket expense will I have?

Most dental benefit plans arise from an agreement between an insurance company and a plan sponsor [employer or union]. There are literally hundreds of plans with endless variables concerning limitations, exclusions, deductibles, annual of lifetime maximum benefits, co-payments and fee schedules. If you have changed jobs, your new plan may not have any resemblance to your old one.

If you are being offered a plan, ask if there are several plans from which to choose. Beside yourself, who in the family will be covered? See if the plan you choose covers pre-existing conditions. If the plan is provided by your union or employer, is there any direct cost to you? If so, how much? Compare this with what you understand the benefits of the program to be.

If you already have dental insurance, we need you to bring in specific information for us to give you the best advice as to your coverage. We must have your insurance policy and ID number. Most important, bring your benefits booklet. If you don’t have one, call your employer or insurance company and request it.

Once we read the benefits book, we can project a general idea for what services and treatment are covered and the percentage of reimbursement. Many third party payers request a “predetermination of benefits” on treatment plans. We can submit the proper form and appropriate records to the insurance company. They will return it with the accepted, allowable treatment and the exact amount of coverage.

Once we have reviewed your plan, we should be able to answer most of your questions. Some plans will only allow the least expensive way to treat a problem, regardless of your choice or our recommendation. We would hope that you don’t base your dental needs and oral health solely on what benefits your plan allows. Our office will be happy to discuss a variety of financial arrangements that will make it possible to receive the treatment you need, while maximizing your insurance benefits.

Eating Right For Your Dental Health

What we eat is significant to our overall health. General guidelines should include balance and moderation and should involve choices from the five major food groups

Dairy [milk, yogurt, cheese]

Meat [poultry, fish]

Fruits

Vegetables

Whole grain [breads, cereals]

What do we do to prevent this potential problem? Avoid having sugar in your mouth for long periods of time. Stay away from sucking candies and chewing gum [unless sugarless] and refrain from drinking soda pop regularly [unless diet]. Try to cut down on the number of snacks per day. If you must snack, substitute foods that most like but don’t promote tooth decay. Examples are popcorn, pretzels, fruits, nuts, cheese, pizza and vegetables.

Consuming sugary foods with a meal or for dessert has a less detrimental effect, because increased salivary flow during meals helps to wash the food away. Also, it is usually nearer the time that most people will brush their teeth. Keeping this in mind, it is better for children to eat sweets at a time and place that allows them to brush soon afterwards.

A balanced diet is also important to keep your bones and gums healthy. Foods rich in vitamin C [citrus fruits and juices, leafy vegetable, potatoes], B12 [dairy, meat] and Folic Acid [spinach, broccoli] will help strengthen your gums and supporting soft tissue. Of course calcium [dairy, dark leafy vegetables] is necessary for the development and maintenance of strong teeth and bones. If you are lactose intolerant, calcium supplements are readily available.
